• ID (driver's license or state ID)

• Insurance card (front and back)

• Office Policy and Registration Form (download)

3. On the day of the visit

• Be on a computer or mobile device with a good internet connection and video camera​

• Have your previous lab/test results ready

• Have a list of your medications and pharmacy information ready

• Find out a lab (LabCorp) location near you - we will need its fax number if we order new labs

• If English is not your primary language, please make sure to have an interpreter with you
